Follow-Up Offer at Surrey Teaching Centre
Aims
- To follow up pupils who have finished their rehabilitation programmes at The Children’s Trust to ensure they are accessing an educational placement
- To offer Brain Injury Education (BIE) alongside The Children's Trust Psychologists
Services
- Provide training/information on the educational implications of an acquired brain injury. This may be alongside educational psychologists or therapists within the team at TCT
- Attend relevant meetings including EHCP reviews as requested / possible
- Advise on adapted resources and support materials if appropriate
Transition work
- Teachers will write Initial Education Reports outlining strengths, needs, and effective support strategies. These reports will be updated at the point of discharge.
- Meet or make contact with teachers/SENCOs of next educational placements if possible
- The Class Teacher or Follow Up Lead will contact the pupil’s receiving school before or soon after discharge if a school has been identified to arrange Brain Injury Education
- If a school has not been identified before the child leaves The Children’s Trust we will keep in touch with the LA and/or parents so that we can offer support once the child has a school placement
Factors affecting the level of transition support
- Support will only be offered to schools with parental/carer consent. If consent is not given, a single check with school / LA will be made to ensure that the child is receiving schooling.
