Follow-Up Offer at Surrey Teaching Centre

Aims

  • To provide transitional advice and guidance for pupils who have finished their rehabilitation programmes at The Children’s Trust, to ensure they are accessing an educational placement and being supported effectively in school
  • To offer Brain Injury Education (BIE) alongside The Children's Trust Psychologists

Services

  • Attend relevant meetings including annual reviews as requested / if possible for up to 12 months following discharge
  • Advise on adapted resources and support materials if appropriate

Transition work

  • Teachers will write Initial Education Reports outlining strengths, needs, and effective support strategies. These reports will be updated at the point of discharge.
  • Teachers will meet or make contact with teachers/SENCOs of next educational placements if possible
  • STC will contact the pupil’s receiving school before or soon after discharge if a school has been identified to arrange Brain Injury Education
  • If a school has not been identified before the child leaves their rehabilitation placement we will keep in touch with the LA and/or parents so that we can offer support once the child has a school placement

Factors affecting the level of transition support

  • Support will only be offered to schools with parental/carer consent. If consent is not given, a single check with school / LA will be made to ensure that the child is receiving schooling.
  • The level of need of the pupil
